This check reports the number of active and failed connections. You can configure levels using the rule "Database Connections". (Note that for MySQL flexible servers Azure speaks of "aborted" rather than "failed" connections.)

Discovery

One service is created for each database if any of the required metrics (average_active_connections, total_connections_failed, total_aborted_connections) are given.
